All items must be submitted by 5:00 pm. on the deadline date for funding consideration. Include nine complete copies, plus the original. Ten total. Each complete grant application must contain each of the following seven components: Please use your checklist.
1. A cover letter of no more than two pages. The letter should include:
- Mission/Purpose of organization
- Funding amount requested
- Describe what the project/program is and what it is to accomplish
- Expected results and outcomes
- Clientele targeted
- Geographic area of service
- Ability to provide long-range planning/funding beyond funding period
- How are you measuring your results
2. Completed Grant Application – Organization Fact Sheet
3. List of Board of Directors and the principal staff of the organization
4. Total budget for proposed project
5. List of additional sources of funding committed or pending to the project
6. Optional-Up to three letters of support
7. Optional-Up to three news articles or print pieces regarding your initiative
8. IRS letter (from the Dept. of Treasury) as proof of 501(c)3 status
9. One copy of most recent audit, if available, or previous year’s financial statements
10. Provide written estimates from one or more vendors for large purchases related to the project.
Emergency and critical needs grants may be considered outside the normal grant cycle.
